Cancellation Policy

 

At Chi Massage Therapy, I understand that unforeseen circumstances can arise in life. In my commitment to maintaining fairness and efficiency for all clients as well as myself, I have established the following policies:

 

Appointment Reminders: A helpful email reminder will be sent 72 hours prior to your scheduled appointment, followed by a text reminder 48 hours in advance. Please note that the text reminder serves as your final opportunity to cancel without incurring a fee.

 

Cancellation Notice:  Your appointment time is exclusively set aside for you. A late cancellation or missed visit creates a gap in the therapists' schedule that could have been utilized by another patient. To ensure fair scheduling and adequate compensation for our therapists, we kindly request a minimum of 48 hours advance notice for any appointment cancellations. Patients who give 48 to 24 hours notice will incur a $150 cancellation fee. For cancellations made 24 hours or less notice, 75% of the treatment cost will be collected, 2 hours or less notice the full rate will be collected.  Late cancellation fees will be charged to the credit card on file. We appreciate your cooperation and understanding.

 

No-Shows: Clients who miss their scheduled appointments without prior notice will be considered "no-shows." A fee of 100% of the treatment cost will be applied to compensate for the missed appointment.

 

Late Arrivals: If you arrive late for your session, please understand that the duration of your treatment may need to be shortened to accommodate other clients with subsequent appointments. Your therapist will evaluate the remaining time and determine if a full treatment can still be provided. Regardless of the actual length of the treatment, the payment for the originally scheduled session will be required. I kindly request that you plan accordingly and arrive on time as a gesture of respect towards your therapist and fellow clients.

Liability Waiver

 

I, the undersigned, hereby acknowledge my desire to participate in the massage therapy treatment provided by the licensed massage therapist associated with Chi Massage Therapy LLC, located at 718 De La Vina Street, Santa Barbara, CA 93101. In consideration of my participation, I affirm the following:

 

Understanding of Treatment Purpose: I comprehend that the massage therapy treatments offered are aimed at supporting healing and promoting relaxation. However, I acknowledge that these treatments should not be considered as a substitute for medical treatment of any illness, injury, or medical condition. Consequently, before commencing the treatment, I will consult with my regular physician. Additionally, I commit to maintaining continued consultation with my physician throughout the course of treatment, seeking guidance on my health and any necessary medical treatment.

 

Acknowledgment of Therapist's Role: I am aware that Brent Kee, as the massage therapist at Chi Massage Therapy LLC or Chi Massage Method LLC, does not function as a physician, nurse, or emergency medical technician. I understand that the availability of these treatments does not entail any responsibility for the management of my medical condition. In the event that my medical condition undergoes any changes, I am solely accountable for discontinuing the treatments and promptly consulting with my physician to assess the suitability of continuing or resuming participation in the treatments.

 

Permission for Treatment Modalities: I grant permission for the practitioner to utilize treatment methods that may extend beyond the typical scope of practice of a massage therapist. These methods may include but are not limited to, the use of electrical stimulation devices such as Scenar/Cosmodic/ENS. Furthermore, I authorize the practitioner to administer chiropractic-like adjustments, which may involve ballistic adjustments and Kinetic Shockwave Therapy, as deemed necessary for my treatment. Lastly, I permit the use of vibration plate therapy as part of the treatment process.

 

I acknowledge that my decision to participate in the treatments is entirely my own and that I have not relied on the advice or representations of any individual associated with Chi Massage Therapy LLC or Chi Massage Method LLC, or any other person, in making this decision.

 

I confirm that I have had the opportunity to seek clarification regarding the treatments, as well as this consent, waiver, and release and that all my questions have been satisfactorily answered. I understand the potential risks involved in participating in the treatments, including the risk of physical injury. I acknowledge my right to decline any aspect of the treatment at any time and to discontinue the treatment entirely if I wish.

 

I accept full responsibility for any and all risks associated with my participation in the treatments and agree to hold Chi Massage Therapy LLC or Chi Massage Method LLC, Brent Kee as an individual, its owners, directors, officers, parents, subsidiaries, affiliates, and agents harmless from any claims, demands, personal injuries, costs, expenses (including attorneys' fees), or damages that may arise, directly or indirectly, from my participation in the treatments.

 

By booking online, I confirm that I have read and understood this consent, waiver, and release, or that it has been read to me if necessary. I voluntarily execute this consent, waiver, and release, fully accepting its terms and conditions.

 

 

Covid Protocol 

 

At Chi Massage Therapy, my utmost priority lies in maintaining a safe and conducive environment for bodywork, health, and wellness. As part of our commitment to implementing the Chi Massage Method, I diligently adhere to all government and CDC mandates, directives, and guidelines. Moreover, we have implemented several additional measures to enhance your sense of safety and relaxation during your time on the massage table.  In addition to fulfilling the legal requirements, I go the extra mile by thoroughly sanitizing the oil bottles, oil bottle holsters, and every component of the face cradle after each client. These items are then set aside and not used again for a minimum of 24 hours. These additional precautions are designed to optimize your comfort and peace of mind throughout the entirety of your massage experience.
