Liability Waiver

I wish to participate in the massage therapy session in association with Chi Massage Therapy L.L.C and the Chi Massage Method at the 718 De La Vina Studio.  As a condition to my participation in the sessions and by booking online, I hereby signify that:

  • I understand that the sessions are offered to support healing and relaxation and to not constitute an effective substitute for medical treatment of illness, injury, or any other medical condition.  I will consult with my regular physician prior to engaging in the session in which I am participating and will continue to consult with such physician during such sessions regarding my health and any medical treatment that I may require.

  • I understand that the massage therapist is not functioning as a physician, nurse, or emergency medical technician and that the massage therapist and Chi Massage Therapy LLC, by making the sessions available, is not undertaking any responsibility regarding my medical condition.  If my medical condition should change, I understand that it is my responsibility to discontinue the session and to immediately consult with my physician about continuing or resuming participation in the sessions.

  • I agree that I am responsible for deciding whether to participate in the session and I have not relied on the advice of any other person, whether associated with Chi Massage Therapy LLC or otherwise, in doing so.

  • I have had the opportunity to ask questions about the sessions and this consent, waiver, and release, and have received answers to my satisfaction.  I understand the risks involved in participating in the sessions, including the potential risk of physical injury.

  • I agree to assume all risks associated with participating in the sessions and agree to assume full responsibility for any injuries, losses, or other damages that I may suffer as a result of my participation in the sessions.

  • I hereby release, indemnify and hold harmless Chi Massage Therapy L.L.C as a company, Brent Kee as an individual, its respective owners, directors, officers, parents, subsidiaries, affiliates, and agents from any and all claims, demands, personal injuries, costs, or expense, including attorneys fees arising from or relating to anyway to my participation in the sessions.

  • I have read this consent, waiver, and release or have had it read to me, if necessary, and I fully understand its contents.  I am voluntarily executing this consent, waiver, and release.

 

COVID Protocol

At Chi Massage Therapy I take bodywork, health, and wellness very seriously. Also in implementing the Chi Massage Method, I am abiding by all government and CDC mandates, directives, and guidelines. Plus a few additional steps to help you feel a little more safe and relaxed while on the massage table. In addition to legal requirements, after each client, I am taking the oil bottles, oil bottle holsters, and the entire face cradle components, sanitizing them thoroughly, setting them in an ozone room for 12 hours, and not using them again until at least 24 hours later. These are a few of the additional things I am doing to help make this a more comfortable experience.

Cancellation Policy

I understand that unanticipated events happen occasionally in everyone’s life. In our desire to be effective and fair to all clients, the following policies are honored:

  • You will receive an email reminder 72 hours before your appointment. You will receive a text reminder 48 hours before your appointment. At the time of this text is your last opportunity to cancel without a fee. A FULL 48-hour advance notice is required when canceling an appointment. This allows the opportunity for someone else to schedule an appointment and so our therapist rightly compensated. If you are unable to give us a full 48 hours advanced notice you will be charged a $200 late cancellation fee, to be paid the same day.

  • No-Shows: Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show”. They will be charged 75% for their “missed” appointment.

  • Late Arrivals: if you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment.

 

  • Regardless of the length of the treatment actually given, you will be responsible for payment of the “full” session. out of respect and consideration to your therapist and other customers, please plan accordingly and be on time.
